These reports were filed between March 1 and May 31, 2026.

TL;DR Version: The top scores went to Croasdaile Village Assisted Living and Eno Pointe Assisted Living, both scoring a perfect 100. The facility with the lowest score was University Health and Rehabilitation with a score of 88.5, reflecting several issues including walls and floors in many locations in need of repair or cleaning, showers needing repair, and hot water at most sinks running above 120F.
